This  Acquired Brain Injury(ABI) case management post will be to join the existing band 8a Case Manager and Consultant Neuropsychologists to triage organise, review and  plan discharges  from  private  rehabilitation providers primarily in Cheshire and the North West . Applications from a nursing or AHP background with extensive knowledge of stroke and brain injury rehabilitation in either the acute or community sector would be accepted. This Rehabilitation and Placement Service ( RAPS) is a  well established model that has been shortlisted for a HSJ Award.

The RAPS sits within the wider community ABI Service of 16 staff  that consists of Vocational Occupational Therapists, Case Managers , Consultant Neuropsychologists, Clinical Psychologists and Assistant Psychologists. The office base is at the Countess of Chester Health Park, Chester but the post holder will travel widely across Cheshire, the North West and surrounding area. A small  community caseload for initial screening assessments and on going case management would also be expected.

The service has regular student placements, opportunities for audit and research and group interventions. 

This is a Three year rolling contract.
