An exciting  opportunity has arisen for two qualified , experienced mental health nurses to join the Primary Care Mental Health Team.  The team covers Central Cheshire and works across South Cheshire and Vale Cheshire ,

The Gateway Service is the single point of referral for mental health services for Vale Royal and South Cheshire.  The post holder will be providing nursing assessment's and planning on going care to patients within a primary care setting, the role involves close working relations with colleagues in both primary and secondary care mental health services.

There is an expectation that staff  will  work out in our local GP practices as well as on occasions in one of our two bases.

You must be able to work between the hours of  8am-8.30pm 7 days week . Although the core service hours are 8am-8pm 5 days week.   There are 2 Part time posts, one 30 hours per week and one  22.50 hours per week however we will consider a full time and a part time post.

This is a band 6  post working with in a busy primary care service.
